Gym membership form format for India: what to include, and a free printable form
By Himanshu Thakur, Founder, Jimwala · Updated · 4 min read
A good membership form protects you and the member. It tells you who the person is, how to reach them, what they paid for and whether they have a health condition you should know about. A bad one is a half-page that is half empty.
Member details
- Full name, and father's or husband's name if you keep it.
- Date of birth, so you know if a member is under 18 and can send birthday wishes.
- Mobile number on WhatsApp. This is the field that matters most, because every reminder goes there.
- Address, for your record.
- Emergency contact name and number.
- A passport photo, or take one on your phone.
- An ID number only if you really keep one. It is optional, and collecting more than you need is a liability.
Plan and fee
Plan, fee, amount paid, anything still due, start date, expiry date, and how it was paid. You will look at this part the most. Without it, arguments about dues begin.
Health declaration
A short tick-list: heart or blood pressure, diabetes, asthma, back or joint injury, recent surgery, pregnancy. It lets the member tell you what you need to know before you give them a program. Add a line saying they have answered honestly and train at their own risk. For proper legal wording, ask a local lawyer. A form does not replace a doctor's advice.
Rules and signature
Keep the rules to five short lines: fees in advance, shoes and towel, care of equipment, lockers, listen to the trainer. End with the member's signature, yours, and the date.
Print it, or skip the paper
Our free gym membership form lets you add your gym name, choose the sections and print it or save a PDF.
The catch with paper is that someone has to type it into the register afterwards, and then the paper gets lost. With a join link or QR poster, the new member fills in the same details on their own phone and you approve it with one tap. It lands in your member list with the expiry and reminders ready. That is what Jimwala does, free for 14 days with no card.
